Release of Liability
I do hereby release and discharge the Baring Cross Baptist Church and its authorized representatives and staff from all liability of any kind and character upon any claim, demand, or cause of action which might be asserted in behalf of said minor against said church, representatives, or staff. Furthermore, in the event of and accident on or off church property or during transportation provided by staff or representative, if the said staff or representative is unable to contact the parent(s) or guardian(s), we hereby grant permission to said staff or representative to administer necessary first aid, and/or take the participant to the nearest medical facility or additional treatment.
